Work to help people who are in an inpatient setting follow-up with discharge planning. Participate in hospital discharge planning, and help members engage in after care services. Work is in the hospital and community settings. Work closely with hospital staff and behavioral health staff to promote health and wellness of members. Provides highly individualized services in the community and promotes individual self-determination and decision-making to individuals with Mental Health or Substance Abuse issues. Must have a valid NC driver's license to qualify.  Must have NC Certified Peer Support certification or pending status.  Competitive wages, benefits, paid time off, holidays, mileage reimbursement.  Clinical supervision provided.  Location, Asheville NC.
